Title is pretty self explanatory.
Breaking it down though...this could counter turtling on some maps (by which I basically mean 2fort) and encourage more people to push as well as go medic. The reason people become apathetic in these situations is because a map is plain broken, but you can introduce gameplay features to counter that problem somewhat. Such ways already exist like the quick fixes ability to jump a medic onto battlements (or anywhere), but so many players are ignorant of it.
For example: say you have people trying to push from a sewer base, having 2 way teles would let them head right back to spawn and get metal quicker without having to use the painfully slow Eureka effect. And medics could res people on the spot, rather than hope they make it back, by which time the base would likely be overrun. A charged quick fix can res a person to full hp in one second flat on MvM. It would make playng these classes far more interesting, as well ending up with better balanced teams.
As for the respawn timer it could just work like MvM and a player can refuse to be ressed by respawning themselves, or have about 15 secs after which they respawn anyway. This shouldn't matter whether it's an instant respawn server or not, if it is you just click to respawn.
I think revive should have been put into PvP by Valve as soon as they made it, as it fits the games class based mmo feel a lot better anyway. Nearly all games like that have revive.
