When was your worst time when a restaurant/fast food messed up your food?

Haven't gone back to a Dairy Queen since the incident of 2016. We don't talk about the incident of 2016.
I once went to an eatery for my lunch meetup with my work colleagues. Placed my request for a certain dish entree. After 2 bites in, it was seeming okay BUT on the 3rd bite, something of unusual texture was rather odd in my mouth. To my horror, I spat out someone's band-aid. I complained to the wait staff and management. Needless to say, I got my lawyers involved and won my lawsuit.
I know the place you're talking about! I once ordered a pizza and a diet dr.pepper from them. Took them 14 hours to deliver and forgot my drink. How was I supposed to enjoy my pizza without my drink?!
My local wendys has a pretty wacky repuation of fucking up orders almost every time you order something. Mainly because its always super busy. But I think the worst meal I ever got there was when they completly forgot to give me a burger and it was just a small fry and drink. Somehow the burger fast food place forgot to give me a burger.

I once saw this guy order there, and they forgot the pickles! At least they gave him a refund.
The closest mcdonalds to me is so bad with orders that when it got remodeled, they put in a third window in the drive-thru specifically for when they mess up your order.
But worst experience is when the local iHop opened up over a decade ago. Drinks came late and there was this black gunk floating around that nobody was able to figure out. My food was somehow both overcooked and undercooked at the same time and there was hair in my food. When I told the waiter about it, they went to talk to the cook and apparently the cook had said "It was my imagination." They shut down for a myriad of reasons a couple years later.
