Agree.
If you've ever played rail only on say instagib servers, you will also notice its not just about 'aiming' to get good aim.
When those milliseconds count, you have to take everything into account including ping, fps, mouse response, and a whole bunch of cvars.
You will find that most people with really good aim, have tweaked the shit out of the game, although there are exceptions that just have a natural ability for accuracy.
also in regard to the hitbox, heres a vid i found, not sure how up to date it is but it gives you an idea..

If you play on servers with 15 or less ping it is often an idea to enter "sv_lan 1" in the console which will optimise your network settings and turn things like cl_lagcompenstation off.

With regards to aim, when you first play a new game change your sensitivity so you can only turn 360 or 180 degrees in one quick motion depending whether the game requires more moveability or aim.
 
This is why you need tweaking on instagib, and also why its a closed shop really, 95% of all new players just scream "you hacker"
On dm17, i used to hit 50 kills in under 2 minutes....
