Two years ago I was banned because I logged in with an IP address that had been used by a hacker. When I appealed I was told by the person who handled my appeal that because my account was set to private he had no way to verify that I wasn't using a proxy account and they denied my request. At the time I was frustrated so I didn't pursue the matter further. Now, I'm back into playing TF2. I've set my account to public and I'm happy to verify any other information you might need to resolve this.

Full disclosure: When I was banned in 2013 I was frustrated and in my impatience opened two tickets, which I think may have prejudiced my case.​
 
Well your IP is listed as dynamic on Charter but I'm still concerned about your IP's ban history.

Only 4 accounts total have been banned over the past two years. Usually when a dynamic IP is switching customers regularly the list winds up being a lot longer. A ban history like this implies to me that the IP doesn't switch between customers regularly if at all.
